I consent to the processing by the Imago Foundation with its registered office in Wroclaw at Hallera Street 123, 53-201 Wroclaw, my personal data for the purpose of organizing the conference, as well as contact in matters related to its organization, using the data provided by me.

I am aware that providing data in the application form and submitting it is voluntary, but necessary to achieve the purpose of processing. Failure to provide data makes it impossible to apply and participate in the conference. I am aware of my rights under the provisions of law, including the right to withdraw consent and that its withdrawal will not affect the legality of actions taken until consent is withdrawn. I have also been informed about the principles and means of data processing and other elements of processing my data contained in the information obligation.

Granted consents are valid now and in the future, provided that the purpose of processing is not changed, objections to the processing of personal data, requests to delete data, requests to stop processing data or withdrawal of consent.

• in order to organize the conference, the Imago Foundation obtains personal data from interested persons in the scope indicated in this form.
• I am aware of my rights under the provisions of law, including rectification of data, objection to data processing, requests to delete data from the ISBA database, as well as the right to withdraw consent at any time and that its withdrawal will not affect the legality of the actions taken during the consent period. If this right is exercised, participation in the conference will not be possible. I have also been informed about the principles and means of data processing and other elements of processing my data contained in the information obligation.
• participation in the conference is tantamount to expressing voluntary consent to the recording and use of the image, voice and statements by the conference organiser, co-organisers and media - in the form of audiovisual recordings and/or photographic documentation - as well as the use of any materials and works on which the Participant's image will be recorded for non-commercial purposes: promotion and communication, documentation and archiving, as well as possible settlements with public and private entities related to project management.
The Participant grants the above consent free of charge, thus waiving any claims (existing and future), including any possible remuneration, against the Participant for the use of their image/voice/statements for the purposes specified above.
The data administrator is the Imago Foundation with its registered office in Wroclaw, at Hallera Street 123, 53-201 Wroclaw. Personal data (in the scope consistent with the form) are provided by persons who declare their willingness to participate in the conference voluntarily, but without providing them it is not possible to participate in the conference. They are collected by the Foundation only for the purpose of their processing for the implementation of the conference. They will be processed on the basis of consent, during the period of its validity, they will not be subject to further processing or profiling, and they will not be based on automated decisions. The data will not be made available to external recipients. The data subject has the right to access data, the right to rectify, delete or limit processing, as well as the right to object to data processing, the right to transfer data and withdraw consent at any time, which will result in the removal of data from the database of conference participants, and has the right to submit a complaint to the supervisory authority.
